fix(support): create missing support_approvals table on migrate
Use SUPPORT_GMAIL_SUBJECT_PREFIX so tickets are identified by subject line without manual inbox labelling. Expose effective_poll_query in poll and setup-check output. Co-authored-by: Cursor <cursoragent@cursor.com>
Align code default with production env (SUPPORT_GMAIL_SUBJECT_PREFIX=codeweek-support). Tickets must include that text in the subject; no Gmail label required. Co-authored-by: Cursor <cursoragent@cursor.com>
